Maintenance not Carried out as Per PMS

Maintain of the machinery to be carried out regularly as per PMS or by continuous monitoring. After trying out the main engine, found everything normal, engine responded for both ahead and astern direction. But when engine was started during manoeuvring, it was observed No 1 cylinder head was lifting. On investigation found the starting air valve stuck in open position.
